1. Original copy of PSA Live-birth Certificate
2. Original copy of Grade 10 Report Card (with Learner Reference Number – LRN)
3. Certificate of Junior High School Completion
4. Original copy of Good Moral Certificate
5. Original ESC Certificate of Letter from JHS Principal; format available through the ESC IMS (ESC Grantees Only)
6. Original QVR Certificate (Qualified Voucher Applicants only/Non-ESC grantees)
7. 4 copies of 2×2 colored ID picture within the last 6 months
8. Photocopy of vaccination card or certificate (full-vaccination)
9. Chest X-Ray Result

From Public Junior High School:
